Tis the Season to be Jolly? Your Health Matters Leslee B. Cochrane, MD Executive Medical Director The holiday season is here, and despite the spirit of good cheer it brings, the darker days of winter can trigger a variety of adverse health-related issues for some people. Perhaps the most widely recognized disorder associated with the winter months is a type of depression known as Seasonal Affective Disorder (SAD). Seasonal Affective Disorder, or SAD, is triggered by a decrease in sun exposure, which typically occurs in late fall and winter. It is estimated that 4-6% of the general population may suffer from clinically significant SAD symptoms, and an additional 10-20% may experience mild symptoms. SAD is four times more common in women than in men and usually affects people older than 20. The condition appears to be directly related to the reduced sunlight of the winter months, and it is therefore not surprising that it is seven times more common in Washington State than in Florida. Common symptoms include feeling sad, loss of interest in activities, low energy and fatigue, excessive sleeping, difficulty concentrating, and increased appetite with cravings for carbohydrates and subsequent weight gain. Effective therapies include cognitive behavioral therapy, light therapy with prescription light devices, and, in some cases, the judicious use of antidepressant medications. Another perplexing condition related to the holiday season is what I affectionately refer to as "the battle of the bulge" that tends to appear around the waist during this time of year. People are more likely to gain weight in the fall and winter and then lose it in the spring and summer. Although there are no unifying theories to fully explain these seasonal variations, scientists have discovered that a lack of sun exposure affects several important hormones and neurotransmitters, which may help explain these adverse health effects. If you are experiencing any seasonally related health challenges, please talk with your doctor for further guidance. In addition to the treatments for SAD mentioned above, the lifestyle modifications below have been shown to improve overall health and help combat seasonal health conditions: 1. Exercise regularly - It releases endorphins, promotes wellness, and reduces stress. 2. Sleep well - Quality sleep strengthens and supports your immune system. 3. Avoid excessive exposure to blue light from cell phones, TVs, and LED lighting - Blue light can impair melatonin production, which in turn disrupts healthy sleep patterns. 4. Unplug from stress and connect with friends - It can do wonders for your mental and emotional well-being. I hope this information helps you enjoy a very Merry Christmas and a Happy New Year!

Civil Bells Mike Patton Bereavement Counselor/Spiritual Care I know what you are thinking that I have lost my mind in the flurry of the season that "silver bells" is what I meant to write. It is true that I love that song for the timeless introduction to the holiday season. So let me explain my reasoning for the twist of terms. As my television preferences have aged, like a fine wine, I have become a bit of a PBS nerd. I enjoy hearing the thoughts of women and men who are well studied and honest, being interviewed by persons of equal intelligence and journalistic integrity. I have come to savor PBS when my body settles into the recliner after the work of the day is done... at least until my TV viewing must be shared. I was watching an episode of Firing Line with Margaret Hoover recently that replayed a 2020 interview she did with Dr. Robert George and Dr. Cornel West, two professors who teach a class together at Princeton University. They are from opposite sides of the political spectrum. Dr. George is a conservative legal scholar and Dr. West is a progressive philosopher and considers himself a non-Marxist socialist. Both men are public intellectuals. Both men are brilliantly articulate and able to defend their thoughts with clear and precise reasoning, and sharply disagree with each other. Both men are Christians and refer to one another as a beloved brother. Their recent book, Truth Matters, builds on the premise that civility in public discourse is a missing ingredient in our present day democracy. Their teaching together models this missing ingredient. They share that the love they have for one another allows them to discuss with honesty their opposing views and "disagree agreeably." In other words, their love allows for serious intellectual debate resulting in deeper solutions to serious human problems. This is refreshingly tasty... like fine chocolate! In our hospice work, we are committed to a non-judgmental interaction with our patients and their families. Our roles, from bathing to medicating to praying, keep their best interest in mind. We respectfully keep our personal opinions to ourselves. This is different from the civility that Drs. West and George are promoting, but it bears similar fruit because it is founded on the principle of love. The team of hospice professionals that I get to work with have exercised this civility consistently and professionally. Civility tolls through our work like "silver bells" reminds us of the joys of the season.

Hospice of the Valleys celebrates National Family Caregivers Month The month of November is dedicated to recognizing and supporting the millions of family caregivers across the United States. This year's theme is "Plug-in to Care". This theme emphasizes connection and encourages caregivers to utilize vital support tools, trusted networks, and educational resources to make their journey less overwhelming. The Day of the Dead Mike Patton Bereavement Counselor/Spiritual Care I have been a public radio nerd for some time now. Last month the NPR station that I listen to had a segment of 'ofrendas' recorded by the public and shared on the radio. These tributes were personal, heartfelt, and inspiring to hear. The segment was associated with Dia de los Muertos (Day of the Dead) celebrated annually on November 1st and 2nd, popular mostly in the US and Mexico, but not exclusively. The ofrenda is an altar created by the deceased's family to honor and invite their spirit home. Food, flowers, and other items identified with the deceased, are placed on a table to welcome them back home for the holiday. The radio ofrendas were stories, some funny, some serious, and all, shared by loved ones. The holiday has roots that date back as far as 3,000 years ago, originating with the indigenous Aztec practices of honoring the dead by placing offerings at tree stumps. In the 16th century, Spanish colonizers moved the holiday to November 1 and 2 to coincide with the Catholic holidays of All Saints' Day and All Souls' Day. It is fascinating to consider how ancient traditions evolve into our present day practices. I honestly attribute my fascination with Dia de los Muertos to families I have served, and Disney's Coco! Honoring the dead is what every person who has ever attended one of our bereavement groups does. The stories shared are in respect of the grief they are experiencing, and honor the deceased. While the ceremonies and beliefs are varied, the love that connects the living to their deceased person is undeniable. It is this thread that unites all grievers and why I am energized by their stories. Our Fall Memorial service, the Love, Light and Remembrance Luminary service (this year on November 18 at Wilson Creek Winery), also holds the power of connection through honor. We gather with family and friends around fun foods and drink to make luminary bags and share a ceremony of remembrance, honoring each loved one with a special word or phrase chosen by the family and read publicly. While limited in scope, the practice is full of meaning and an effort to honor the dead. Hospice care is often followed by bereavement care. We are aware of the grief that attends death and hold the belief that while death may end a life, it does not end at relationship, similar to the beliefs celebrated on Dia de los Muertos.

The Health Benefits of Gratitude Leslee B. Cochrane, MD Executive Medical Director Thanksgiving is an ideal time to reflect on our many blessings and express our gratitude in meaningful ways. Multiple studies have demonstrated health benefits associated with having an attitude of gratitude. In 2023, The National Library of Medicine published an article summarizing the results of 64 randomized clinical trials studying the effects of gratitude on health. The study demonstrated that patients who participated in various forms of gratitude interventions experienced greater feelings of gratitude, fewer symptoms related to anxiety and depression and better overall mental health. Other studies have shown additional benefits of gratitude including improved sleep quality and a reduction in markers for the risk of cardiovascular disease. A large study published in 2024 in the Journal of the American Medical Association - Psychiatry, looked at the health benefits of gratitude on a group of 49,275 women enrolled in the Nurses' Health Study. The study found that those participants who demonstrated higher levels of gratitude had a 9% lower risk of death over the four year observation window when compared to those with low levels of gratitude. Although the study was not able to determine exactly how higher levels of gratitude impacted overall health, this was the first major study to show that having an attitude of gratitude may improve your life expectancy! If you would like some practical suggestions on how to improve your sense of gratitude, see the below recommendations offered by Maureen Salamon, Executive Editor, Harvard Women's Health Watch from an article she wrote[i] Six questions to evoke gratitude Not feeling especially grateful today? You have the power to change that. Asking yourself certain questions can evoke gratitude, such as 1. What happened today that was good? 2. What am I taking for granted that I can be thankful for? 3. Which people in my life am I grateful for? 4. What is the last book I read or movie, show, or social media clip I saw that I really appreciated, and why? 5. What am I most looking forward to this week, month, and year, and why? 6. What is the kindest thing someone has said or done lately? Similarly, a few simple actions can infuse gratitude into your days. Try developing a family routine of regularly expressing gratitude around the dinner table. Another well-known practice that's perhaps becoming forgotten in this digital age is penning thank-you notes. Choosing an attitude of gratitude is a very beneficial to your health and has absolutely no adverse side effects. How to Prevent those Blue Light Blues -Your Health Matters Leslee B. Cochrane, MD Executive Medical Director Your amazing brain has a built-in clock located in the superchiasmatic nucleus which helps to regulate your 24 hour circadian rhythm. This internal clock regulates your sleep-wake cycles, hormonal cycles, body temperature and other vital bodily functions. Your brain relies on regular cycles of light and dark to maintain your circadian rhythm. Melatonin is a hormone produced by the pineal gland in the evening and throughout the night during low light conditions which helps your body relax into a healthy and regular sleep rhythm. Night time exposure to even small amounts of light is enough to disrupt this delicate system. Disruptions in the sleep cycle have been shown to increase the risk of numerous serious health conditions including heart disease, diabetes, weight gain, depression and anxiety. Natural sunlight exposure during the daytime with its balanced spectrum is beneficial and helps to boost your mood, regulate your circadian rhythm promote Vitamin D production. Artificial light which tends to be in the blue wavelength and includes common sources such LED lighting, TV and phone screens does not have the same beneficial properties as natural sunlight and in fact has several negative effects. In particular, cell phone use near bedtime causes difficulty falling asleep and poor sleep quality which results in excessive daytime sleepiness and increased rates of anxiety and depression. Although no one has answered the question of what is a "safe" amount of screen time, the consensus seems to be less than 2 hours per day for adults and less than 1 hour per day for children. In addition to disruption of the circadian rhythm, excessive daytime exposure to screens tends to cause eye strain, dry eyes and increases the risk of cataracts and macular degeneration. Night time exposure to even small amounts of blue light has been shown to interfere with healthy sleep cycles which in turn can cause a myriad of health concerns. Here are a few suggestions to help you to avoid the "Blue Light Blues": - Open the blinds and use natural light during daytime to reduce your exposure to blue light - Limit screen time to under 2 hours per day for adults and 1 hour per day for kids - If you must work with a screen for longer periods, use blue blocking glasses or screen filters - Decrease screen brightness to a lower setting which will reduce the intensity of the blue light - Avoid exposure to screens at least 3 hours before bedtime and set your device to night mode - Use an eye mask to reduce light exposure at night In our modern world it is all but impossible to completely avoid exposure to blue light; but with a few modifications to your daily routine you can dramatically reduce your exposure to blue lights, thereby avoiding the "Blue light Blues."

The Honor of Serving Mike Patton Bereavement Counselor/Spiritual Care As we recognize the 40th Anniversary of Spiritual Care Week (October 19-25) I want to publicly thank my colleague and friend, Fred Schaeffer, for sharing in this honorable service with me. We are very fortunate to serve an organization whose mission is consistent with our calling as spiritual care providers. Hospice of the Valleys not only encourages spiritual care support to each of our patients and their families, but also values the input and support of our services to our families and our marvelous staff and volunteers. At our October staff meeting I said, "On behalf of Fred and myself, we commit ourselves to your service - to our collective service - that distinguishes Hospice of the Valleys as a 5-star organization committed to reverence for life, relief of suffering, and compassion in loss for our patients and families. Thank you." The consistency of mission makes it very easy for us to serve with honor. The ruby is the symbol for this 40th anniversary, a sign of protection that sustains a good and whole life. I read that under ultraviolet light, the ruby shines to reflect an inner light that in other situations is difficult to see. It is a good symbol for spiritual care as spiritual care services often brings clarity and hope in the end of life journey. The spiritual care providers' goals are consistent with the mission of reverence for life and relief of suffering. It is a rare occasion but I was recently present to witness a hospice patient take her last breath. Her spouse, a dear friend and retired pastor, arrived shortly after and we got to sit together and reminisce a beautiful relationship. I could not help but think of how honored it felt to hold this sacred space with him. Even in the shock of the news of her death, he was able to express his deep love and appreciation for the gift she had been to him for nearly 60 years! I recall that the last words she heard were, "you have been such a blessing to your husband." For all of you who have encouraged Fred and myself whenever our paths have crossed, thank you! It is an honor for us to serve as spiritual care providers and a privilege to know you. The offering of spiritual care services continues to be our honor.

Remembering 1/11 On September 11, 2001, our nation changed forever. We pause to honor the lives lost on that tragic day, to remember the courage of the first responders who rushed toward danger, and to hold in our hearts the families who continue to carry the weight of that loss. We also remember the military families who have borne the cost of service in the years since, and the many service members whose sacrifices were made in the wake of 9/11. Their courage and commitment continue to protect the freedoms we hold dear. As a community hospice, we understand the importance of remembrance, reflection, and honoring those who have gone before us. On this anniversary, we extend our thoughts and prayers to all who were touched by 9/11 and recommit ourselves to living with compassion, gratitude, and service. May we never forget.

National Cholesterol Awareness Month Leslee B. Cochrane, MD. Executive Medical Director September is National Cholesterol Awareness Month - a good time to review why adults should regularly check their cholesterol levels. Most people associate cholesterol with bad health outcomes; but that is not entirely true. Cholesterol is actually an essential building block necessary to build cell membranes, create bile for digestion and required to synthesize hormones and Vitamin D. However, too much cholesterol can lead to a build-up of a waxy plaque in your arteries which in turn can cause various complications including heart attacks and strokes. To better understand cholesterol and related fats (lipids), we can divide them into three different categories: the good, the bad and the ugly. HDL or High Density Lipoprotein Cholesterol, is considered the "good" cholesterol; primarily due to its ability to help protect your arteries by removing plaque build-up from within the blood vessels. LDL or Low Density Lipoprotein Cholesterol, is considered the "bad" cholesterol because it makes a sticky substance known as plaque. High levels of LDL cholesterol can result in a build-up of plaque and cause blockage in your arteries. VLDL or Very Low Density Lipoprotein Cholesterol and Triglycerides, are considered "ugly" because VLDL Cholesterol transports triglycerides from the liver to store them as fat into fat cells. High levels of VLDL and Triglycerides also contribute to the build-up of plaque within the blood vessels. If you have not had your cholesterol levels checked recently, I suggest you consult with your health care provider to have a full lipid panel done. It is important that you plan to fast for approximately 12 hours before the test in order for the results to be considered accurate. Below is a summary of the items typically included in a lipid panel with results categorized as "Dangerous", "At-Risk" and "Heart-Healthy" by the Cleveland Clinic[i]. The main items included in a lipid panel are: Total cholesterol. This is the total amount of cholesterol that's circulating in your blood. LDL or low-density lipoprotein. This is the "bad" cholesterol that contributes to plaque buildup in your arteries, leading to heart attacks and strokes. HDL or high-density lipoprotein. This is the "good" cholesterol that helps clear out the cholesterol your body doesn't need. Triglycerides. This is a type of fat that can raise your heart attack and stroke risks.

Signs of Strokes - SOS! Leslee B. Cochrane, MD. Executive Medical Director Although SOS does not actually stand for "Signs of Strokes"; it is an international code indicating extreme distress and so it seems fitting to use it in reference to strokes. Most people are unaware about the connection between strokes and dementia. Alzheimer's disease is recognized as the leading cause of dementia in the US; but vascular dementia (frequently caused by strokes) is the second leading cause of dementia, accounting for 15-20% of all dementia cases. The underlying cause of strokes is cerebrovascular disease which is a disease affecting the blood vessels in the brain. The term TIA stands for "transient ischemic attack" and refers to a condition in which the blood flow to a portion of the brain is temporarily blocked. TIA's are sometimes referred to as "mini-strokes" because they produce similar symptoms of a stroke; but they are typically smaller and "transient" with most resolving within 1-2 hours. Unlike a stroke where the blockage is permanent, a TIA is caused by a temporary blockage in the smaller s blood vessels and there is less permanent residual damage than that caused by a stroke. Here are some common signs of strokes and TIAS to be aware of: - Weakness, numbness or tingling (usually affecting one side of the body) - Confusion, loss of coordination or balance - Difficulty speaking or swallowing - Sudden loss or blurring of vision Ignoring the signs of a stroke or TIA may lead to permanent neurological damage and could result in death. It should be noted that 10% of patients who have a TIA will later go on to have a stroke within three months. Half of those who have a stroke will have the stroke within 48 hours of the TIA. The most common cause of a TIA is a blood clot that blocks the blood vessel and prevents the vital delivery of oxygen to the brain cells. There are several risk factors for TIA's and strokes including high blood pressure, smoking. diabetes, obesity and elevated cholesterol. Because it is impossible to distinguish a TIA from a stroke at the onset, you should always seek urgent medical attention at the onset of any of the above symptoms. If caught in the early stages, there are treatments available which can reduce the devastating impact of a stroke; but the best strategy is to reduce your risk and thereby prevent the chances of ever having a stroke. Small vessel cerebrovascular disease is a common condition in patients with the above risk factors and may result in functional and cognitive decline including vascular dementia. Patients at increased risk of stroke should consult their health care provider to learn about stroke prevention including diet and exercise. It is also important to discuss treatment options to reduce their risk further by treating conditions such as hypertension, diabetes, elevated cholesterol and obesity. When it comes to strokes the old saying is true: "an ounce of prevention is worth a pound of cure." By recognizing the risk factors of strokes and taking action you can dramatically reduce your risk of stroke.: Conditions That Can Cause Stroke-Like Symptoms Seizures Migraine headaches Dizziness Bell's Palsy Trigeminal neuralgia Neuropathy

Confident Humility Mike Patton Bereavement Counselor/Spiritual Care An image has become very personal for me over my many years of pastoral service. I wish I could say it came from one of the Masters, but it came from Gary Larson's Far Side catalog. The image is of a man, pushing a wheelbarrow while working in the flames of hell. He appears to be quite content, pushing a load of coal through the flames, whistling while he works. The demon overseers provide the commentary, "You know, we're just not reaching that guy." It has become a self-effacing joke that I have told from the pulpit and in front of crowds, whenever it seems appropriate. I share it here to offer a perspective on confident humility. While I poke fun at myself for the "zippity-doo-dah" approach I often take in the midst of challenging circumstances, it is the idea of contentment that best describes my heart and devotion to the work to which I've been called. We are not all called to sit with dying people and their families, finding ways to support each one as they face the end of life concerns, challenges and decisions. The work I get to do as a chaplain and bereavement counselor give me reason to whistle a happy tune every day, and I know that this work is not for everyone. Nevertheless, we all live in community with one another in a world that often seems to be run by... demons! Confidence comes from a heart set and mind fixed on a mission. Our mission at Hospice of the Valleys speaks loudly to my heart, "We foster reverence for life, relief of suffering, and compassion in loss for the terminally ill residents and their families in the communities we serve." From the day I began my service as a chaplain in this organization, my heart aligns with this mission. Everyone I work with has the same mission. Humility is an honest assessment of gifts and abilities, strengths and weaknesses, accepting both responsibility for and limitations to ones involvement in the mission. Obviously, chaplains do not offer medical advice, that would be tragic! However, when I am working in my lane of responsibility, I provide the best care I know how to give, and believe that it contributes to the well-being of the patients and families we serve. I hope that your "image" comes from something other than a Far Side comic. But I also hope that you have what I have in terms of confident humility to serve with joy and contentment in whatever mission you're called to, under whatever circumstances you find yourself. Grab your wheelbarrow... and let's go!

Here Comes the Sun! Your Health Matters Leslee B. Cochrane, MD. Executive Medical Director Summer has arrived and with it comes opportunities for fun in the sun! While enjoying time outdoors, one must take precautions against excessive sun exposure due to the adverse effects of ultra violet radiation, also known as "UV" exposure. July is UV Safety Month and a good time to discuss the risks of excessive UV exposure. The adverse effects of UV radiation are cumulative over your lifetime and can lead to sunburn, premature aging of the skin and increased risk of developing other conditions such as cataracts and skin cancers including melanoma. Excessive sun exposure is the number one risk of all skin cancers. In the United States there are over 5 million people treated with skin cancer each year, making skin cancer the most commonly diagnosed form of cancer. Melanoma is the most dangerous form of skin cancer and there are more than 100,000 new cases of malignant melanoma diagnosed each year. Three Myths about UV Exposure: Myth: I don't need to worry about UV exposure if I am wearing sun screen. Truth: Sunscreens help to protect your skin from UV exposure, but they are only a part of an adequate protection strategy. In addition to a good quality waterproof sun screen, you should also utilize shade, hats, UV protective sun glasses and UV protective clothing for additional protection. Truth: Myth: I don't need to worry about UV exposure if it is cloudy outside. About 80% of the harmful UV rays penetrate through the clouds and are reflected off of water, sand and pavement meaning that you will still need adequate UV protection even on cloudy days. Myth: People with dark colored skin don't need to worry about UV exposure. Truth: Although people with dark skin are less likely to develop skin cancer than people with light colored skin, they are still at risk and when they do develop skin cancer it may be more dangerous as it may be more difficult to detect. 5 Tips for Staying Safe in the Sun: Tip 1: Check the UV Index Use your smartphone to check a weather app before heading outdoors and take a look at the UV index. The UV index is a number rating the UV intensity from 1 to 11. During the summer months, you should avoid direct sun exposure between the hours of 10 am - 4 pm or any time that the UV index is above 6 on the 1-11 scale. Tip 2: Prioritize Eye Protection UV exposure can lead to keratitis (sun-burn of the eye), cataracts and age related macular degeneration. Make sure you wear sun glasses with full UV protection. Tip 3: Slap on the Sunscreen Select a sunscreen with a SPF of 30 or above which blocks both UV-A and UV-B rays. Use a water-resistant sunscreen if you will be in the pool and be sure to apply it 20 minutes before sun exposure and then re-apply every 2 hours while you remain in the sun. Tip 4: Dress for Sun Success Select a hat with a broad brim and neck protection and wear loose fitting sun protective clothing while outdoors. Tip 5: Hydrate or Die-drate Find shade or bring it with you in the form of an umbrella. Remember to stay well hydrated when outdoors which will help your body stay cool and prevent heat related injuries such as heat exhaustion and heat stroke. By observing these safety tips you can safely enjoy lots of fun in the sun this summer!

HONORING THE SPIRIT OF FLAG DAY JUNE 14, 2025 At Hospice of the Valleys, we pause on Flag Day to honor the enduring symbol of our nation's ideals-freedom, unity, and compassion. The American flag stands as a reminder of the courage and sacrifice that have shaped our country, and of the values we strive to uphold every day in the care we provide. Many of our patients and families have deep connections to the flag-some as veterans, others as lifelong patriots who cherish the strength and resilience it represents. On this day, we reflect on their stories and their service, with great dignity as they're faced with life's final chapters. Flag Day is also a moment to appreciate the communities we serve, bound together by shared respect, love, and hope. We are honored to walk beside our patients and their loved ones with grace and compassion, always guided by the principles that make this nation strong. As you see the flag flying this month, may it inspire gratitude for those who came before us, and hope for the kindness we can bring to one another every day.

June is Alzheimer's and Brain Awareness Month Leslee B. Cochrane, MD. Executive Medical Director June is Alzheimer's and Brain Awareness Month and a good time to learn about Alzheimer's disease including the various risk factors and strategies to promote a healthy brain and potentially reduce the risk of dementia. It is currently estimated that there are over 7.2 million people age 65 and older in the United States currently living with Alzheimer's disease. This number is projected to increase to 13 million by 2050. Age is the number one risk factor for developing Alzheimer's disease with about a 1 in 13 risk of the disease for those aged 65-84 with a significant increase risk of about 1 in 3 people aged 85 and older. In addition to age, having a family history of Alzheimer's disease and certain genetic factors like the APOE e4 gene can also increase the risk of developing Alzheimer's disease. In order to help prevent Alzheimer's disease it is important to manage the numerous modifiable health factors which also increase the risk of developing Alzheimer's disease: - Cardiovascular disease - including hypertension, stroke and high cholesterol - Type 2 diabetes - Obesity and poor nutrition - Smoking and excessive alcohol consumption - Depression - Head injury - Hearing Loss - Social isolation and lack of mental stimulation - Physical inactivity In addition to managing the above listed risk factors, the American Alzheimer's Association has recommended the following "10 Healthy Habits for your Brain"[i]: Challenge your mind- Be curious! Put your brain to work and do something that is new for you. Learn a new skill. Try something artistic. Challenging your mind may have short- and long-term benefits for your brain. Stay in school-Education reduces the risk of cognitive decline and dementia. Encourage youth to stay in school and pursue the highest level of training possible. Continue your own education by taking a class at a local library or college, or online. Get moving-Engage in regular exercise. This includes activities that raise your heart rate and increase blood flow to the brain and body. Find ways to build more movement into your day - walking, dancing, and gardening-whatever works for you! Protect your head - Help prevent an injury to your head. Wear a helmet for activities like biking, and wear a seatbelt. Protect yourself while playing sports. Do what you can to prevent falls, especially for older adults. Be smoke-free- Quitting smoking can lower the risk of cognitive decline back to levels similar to those who have not smoked. It's never too late to stop. Control your blood pressure- Medications can help lower high blood pressure. And healthy habits like eating right and physical activity can help, too. Work with a health care provider to control your blood pressure. Manage diabetes- Type 2 diabetes can be prevented or controlled by eating healthier, increasing physical activity and taking medication, if necessary. Eat right- Eating healthier foods can help reduce your risk of cognitive decline. This includes more vegetables and leaner meats/proteins, along with foods that are less processed and lower in fat. Choose healthier meals and snacks that you enjoy and are available to you. Maintain a healthy weight-Talk to your health care provider about the weight that is healthy for you. Other healthy habits on this list - eating right, exercising and sleeping well- can help with maintaining a healthy weight. Sleep well- Good quality sleep is important for brain health. Stay off screens before bed and make your sleep space as comfortable as possible. Do all you can to minimize disruptions. If you have any sleep-related problems, such as sleep apnea, talk to a health care provider.

Conversation with Guilt Mike Patton Bereavement Counselor/Spiritual Care Many years ago, I attended a religious conference for men. One speaker said, "when the enemy of your soul comes knocking at your door to accuse you, just say, "Lord, will you get that!" A great line, easier said than done. In the twenty-plus years that I have been involved in bereavement groups, every group has dealt with the issue of guilt. As a compassionate person, I have developed a good poker face while listening, but when the culprit keeps showing up and harassing the bereaved, I cannot stand it! I have often said that guilt is the cruelest part of the grief journey. A common example is when a member of the group shares that they smiled or laughed and enjoyed a moment, followed by a comment like, "I felt guilty for feeling good." Comments like that are not for me to challenge as much as they are for me to respect. Guilt found a way to interrupt an otherwise beautiful moment of joy. With guilt personified, I suggest one ask, "Are you here to teach me something?" By doing this, a griever is humbly acknowledging the emotion of guilt and seeking reasonable response to it. If, for example, the guilt is connected to a teachable moment, "I feel badly that I didn't visit Grandma the day she died," I can acknowledge the reminder to cherish the people in my life and look for opportunities to connect. However, if the guilt is a repetition of unhelpful put downs, "you're a terrible grandson," then I reject the intrusion with the truth, "I loved my grandma and did the best I could at the time." Conversations are food for healing. A conversation with guilt can offer a right response to an otherwise negative emotion. Learning how to hear from and what to say to guilt can go a long way toward healing and peace... not to mention growth as a human being. I am indebted to the many women and men who have trusted their grief journey to a group of grievers gathered to support one another in one of the most difficult times in their lives. They would tell you to be on your guard for guilt... it may come knocking on your door. When you recognize it, speak to it with the authority to either learn from it, or send it away without regret!

Your Health Matters - Combating Caregiver Burnout Leslee B. Cochrane, MD. Executive Medical Director It is estimated that as many as 53 million Americans will serve as unpaid caregivers for family members with chronic illness and/or disabilities this year. The value of the care provided by these unpaid caregivers is estimated to exceed $450 Billion annually. Caregiving is very demanding work and it taxes the emotional, physical and financial well-being of those providing care. According to a 2023 survey of unpaid caregivers conducted by AARP, more than 60% of those surveyed felt that caregiving increased their level of stress and worry. When this increased stress continues over a long period of time, it can lead to a condition known as caregiver burnout, which is a state of emotional, physical and mental exhaustion. In order to better understand caregiver burnout it is helpful to recognize some of the warning signs. Below is a list of some of the common warning signs of caregiver burnout: 1. Changes in appetite- eating too much or too little. 2. Inadequate sleep and feeling exhausted. 3. Mood changes including anxiety and depression. 4. Self neglect- not attending to medical, emotional and spiritual needs. 5. Danger signs such as elevated blood pressure and chest pains warrant immediate medical attention. Caregiver burnout is a very real and growing problem and the cost of providing personal care is expensive and not covered by most insurance plans. Although there are no easy answers to address this growing problem, there is help available. If you or someone you know is experiencing symptoms of caregiver burnout then please reach out to your healthcare provider for assistance. Below are some additional caregiver resources available in our community which I hope you will also find helpful: Riverside County Office on Aging: Family Caregiver Support Program Family Caregiver Support Program aims to serve adult family members caring for a relative who is 60 or older or relative living with dementia. Services include: Education, Training, and Support are provided through caregiving classes which are offered to caregivers at intervals throughout the year. Classes are available in-person and online in a group setting. Support groups are also available. Information and Assistance: Caregivers may receive referrals and linkage to additional community-based services and public benefits to assist in the caregiving journey. Care Management: Caregivers are provided with individualized care from a designated Social Services Practitioner, to assess their needs as well as those of your loved one, provide emotional support, education, resources, referrals, and direct advocacy to help meet your needs and prevent burnout. Monthly contact is made to ensure care plan goals are met. Respite: To provide caregivers temporary relief of caregiving responsibilities, respite may be provided to case management clients. Respite is dependent on available funding. All services are provided free of charge. The Hospice of the Valleys Special Recognition Awards were presented to Delores Schultz, Larry Sieks and Bob Diaz. The Hospice of the Valleys Care Partners Awards were presented to St. Martha's Quilters and The Colony Yarnsters. Congratulations to our awardees in recognition of their exemplary volunteer service! Our volunteers are the best of the best. They do so much for so many and our community is a better, more compassionate place because of their service. They are an essential part of our team and help form the heart of hospice. We could not do the work we do without each and every one of volunteers, we are truly grateful. What's in a Name? Mike Patton Bereavement Counselor/Spiritual Care While I have spent my entire life on the Protestant side of the Christian faith tracks, the longer I live the more I enjoy learning about and sharing in the faith of my Catholic brothers and sisters. I have felt a tremendous unity with them in this season of grief as the world reflects on the death of Pope Francis. Of his life: his Latin American roots, his faithful service in the Jesuit order and rising through the ranks of the Catholic hierarchy, and his election to Pope on March 13, 2013, we have been reminded. What I find most inspiring is the selection of his Papal name from St. Francis of Assisi. Jorge Mario Bergoglio took the name of Pope Francis and lived into that name so well that in his death the entire world is being reminded of his servant leadership, that he was the approachable Pope, and that his service was characterized as Christ-like, which I believe is the highest and most noble calling for anyone who identifies as "Christian." To be the leader of over 1 billion people, there is much in the name. St. Francis of Assisi's well-known prayer explains it all very well: Lord, make me an instrument of Your peace. Where there is hatred, let me sow love; Where there is injury, pardon; Where there is doubt, faith; Where there is despair, hope; Where there is darkness, light; Where there is sadness, joy. O Divine Master, grant that I may not so much seek to be consoled as to console; to be understood as to understand; to be loved as to love. For it is in giving that we receive; it is in pardoning that we are pardoned; and it is in dying that we are born to eternal life. Contemporary culture perceives servant leadership as being weak and ineffective. Pope Francis turned that notion upside down and inside out. His legacy of humble service with the full power of the papacy in his hands is a remarkable example for all to see, ponder and apply. It is abundantly clear to me that Hospice of the Valleys' leadership operates from a similar playbook, humble service with a deep commitment to providing kind and healing presence to our patients and their families. May humble service always be associated with our good name! "Together with a culture of work, there must be a culture of leisure as gratification. To put it another way: people who work must take the time to relax, to be with their families, to enjoy themselves, read, listen to music, play a sport'.
